Phoenix House is looking for a Full-Time Mental

Health Counselor for our residential treatment program in our Springfield Gardens (Queens)

location.

RESPONSIBILITIES:

  • Provide assistance to his/her caseload using

restorative services to foster a successful discharge to an independent living

status; by meeting with and providing 5-6 services, for each of the restorative

services on their treatment plan, per resident, monthly.

  • Provide ongoing contact with treatment

providers to ensure a seamless participation of services as he/she follow up

with the psychiatrist, therapist, medical team and pharmacy to ensure that all

medications prescribed are received in a timely fashion while making sure

refills are filled properly.

  • If applicable send AOT reports out monthly as

well as follow up with parole or probation officers; including anything Phoenix

House decides is appropriate for this position.

EDUCATION/CREDENTIALS

  • High School diploma or

equivalent

  • CASAC-TEXPERIENCE:

Experience

working with special needs population/mental health clients

KNOWLEDGE/SKILLS/ABILITIES:

Computer

literate with knowledge of MS Word & Excel
