Phoenix House is looking for a Full-Time Intake Counselor for our residential treatment program in our Springfield Gardens location.

Work Schedule:Sunday, Monday, Tuesday 7:00a.m.-3:30p.m., Wednesday and Thursday 1:30p.m.-10:00p.m.


  • Set up intake appointments.

  • Meet with client for screening and assessment to discuss possible intakes during team meetings.

  • Provide assistance to his/her caseload using restorative services to foster a successful discharge to an independent living status; by meeting with and providing 5-6 services, for each of the restorative services on their treatment plan, per resident, monthly.

  • Provide ongoing contact with treatment providers to ensure a seamless participation of services as he/she follow up with the psychiatrist, therapist, medical team and pharmacy to ensure that all medications prescribed are received in a timely fashion while making sure refills are filled properly.

  • If applicable send AOT reports out monthly as well as follow up with parole or probation officers; including anything Phoenix House decides is appropriate for this position.


  • High School diploma or equivalent



Experience working with special needs population/mental health clients


Computer literate with knowledge of MS Word & Excel
