(Full-Time) Reporting to the Deputy Director, the Case Manager is responsible for individual service coordination for clients enrolled in Phoenix House adult Transitional Housing and Residential Services, including supporting a successful transition back into the community. In collaboration with the Program staff, the Case Manager supports the successful integration of medical care, housing and income/ employment assistance, substance abuse treatment, and other services within individual treatment plans.
- Monday – Friday, 9:00AM – 5:30PM
Bachelors in Business, Social Services, or Psychology preferred.
Additional experience may be substituted for an educational requirement, or additional education may be substituted for experience.
Certified by a certified body recognized by the Department of Health Care Services (DHCS) as an alcohol and drug counselor ( highly preferred)
Registered by a certifying body recognized by the Department of Health Care Services (DHCS) as an alcohol and drug counselor prior to date of your hire (required)
- 2-3 years progressive experience working as a case manager, preferably within a drug or alcohol treatment setting or transitional housing environment.
Ability to work in a fast-paced environment and exercises independent judgment and leadership, analyze situations accurately, and communicate effectively.
Ability to write clear and concise reports that utilize data. Must show an aptitude for service and for working as part of a multi-disciplinary team.
Computer literate with demonstrated proficiency in Microsoft Office and web-based applications.
Demonstrated ability to utilize electronic health record management system.
Strong organizational skills.
Must maintain a valid CA Drivers’ License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy.
