(Full Time) Reporting to the Senior Program Director, the Care Coordinator is responsible for the clinical functions of determining resource needs of a caseload of clients and their families and facilitating client and family’s enrollment in medically necessary services before, during, and after discharge from substance abuse treatment. They will provide case management services, conduct assessments and screenings, will advocate for client and client’s family, and will be available for consultation with the service partners via teleconference, phone, and in person. They will also work with the client’s primary counselor/therapist to oversee the integration of other services, such as housing, legal, educational, and vocational services into the client’s treatment episode.

  • Variable – Daytime, Evening, Overnight, and Weekend

REQUIREMENTS:

EDUCATION/CREDENTIALS:

  • Bachelors of Social Work required

  • Master of Social Work strongly preferred

  • Certified by a certified body recognized by the Department of Health Care Services (DHCS) as an alcohol and drug counselor ( highly preferred)

  • Registered by a certifying body recognized by the Department of Health Care Services (DHCS) as an alcohol and drug counselor prior to date of your hire (required)

EXPERIENCE:

  • 2-5 years of experience in a case management setting serving under-resourced populations

  • DMH documentation, EBP Trained/certified preferred

KNOWLEDGE/SKILLS/ABILITIES:

  • Proficiency in MS Office products and experience utilizing web-based programs.

  • Excellent organization and time management skills.

  • Therapeutic Community treatment modality or chemical dependency services experience is preferred.

  • Ability to work efficiently in a fast-paced team environment.

  • Ability to establish and consistently enforce appropriate boundaries with clients.

  • Ability to model positive behavior

  • Must maintain a valid CA Drivers’ License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy.
