Phoenix House is looking for a Part-Time LPN for our Ronkonkoma location. The LPN will provide prescribed treatment and nursing care to patients. Implement doctor's orders, monitor specified medication/refills. Monitor and record vital signs as needed, and maintain medical/nursing records.

Shift: Part-Time Evening: Saturday's & Sunday's, 2:30 p.m. - 11:00 p.m.

Responsibilities:

  • Administer prescribed medications; monitor vital signs and other detoxification symptoms/responses.

  • Coordinate provision of medications with providers and pharmacy

  • Maintain medical/nursing records.

Skills:

  • Crisis management and be organized.

  • Computer skills

Educational/Credentials:

  • Licensed Practical Nurse for New York State

  • NARCAN/Opioid Overdose Reversal

  • CPR/First Aid

Experience:

  • Administration of medications, acquisition of vital signs and documentation of such interventions.

  • Ability to communicate with a multi-disciplinary team

  • Experience with substance abuse patients and the ability to identify signs/symptoms of overdose and withdrawal.
