Phoenix House Case Manager in Brandon, Florida
This position is responsible for providing a wide range of case management services to clients in the Hillsborough County jails who meet criteria for a diagnosis or substance use disorder. This position will work in collaboration with jail and other community partners to link clients to services both within the agency and in the larger community to address the identified substance abuse need and ensure appropriate continuum of care. Extensive networking with local service providers and the criminal justice system to ensure access to services and interagency coordination are additional duties. This position will act as a patient advocate in accessing services and ensuring services are used effectively.
Essential Duties and Responsibilities:
Manages an assigned caseload of patients with substance use disorder who are involved in the criminal justice system. I
dentifies case management needs and develops individualized case management service plans that match the needs with appropriate resources.
Secures, monitors and coordinates multiple services to address identified needs including housing, transportation, general healthcare, vocational resources and financial assistance.
Provides a warm hand off to facilitate engagement of patients with substance use treatment both within the agency and with partnering agencies.
Works collaboratively with community agencies, court and the criminal justice system.
Documents services in a timely manner in the electronic health record and updates the record within set guidelines.
Maintains effective working relationships with co-workers, clients and community agencies.
Assures compliance with agency policy, client rights, confidentiality, and state regulations.
Ability and flexibility to work in a variety of settings.
- Graduation from an accredited college with a Bachelor’s Degree in Human services plus experience providing case management services, or any equivalent combination of education and experience which provides the required knowledge, skills and abilities.
